Please print the application form and either bring it into our office at 410 South 2nd Street Bismarck ND 58504 or mail it to that address. Faxed copies will not be accepted. Please make sure to fill out the application completely or it will be returned to you and will delay the application process. You will not be placed on the waiting list until a completed application has arrived at our office.
