Print the application package; mail it, or bring it into our office at 410 South 2nd Street Bismarck ND 58504.
You will not be placed on the waiting list until a completed application has arrived at our office.

- Additional forms - Debts Owed to Public Housing Agencies and Terminations (Required for each adult.)
- Additional forms - Current and Prior Landlord References. (Required for each adult)
Your application will be returned to you as incomplete without signed copy(s) of these additional forms.
Request for Applicant to be Removed from an Application. (Required for each adult. Must be notarized.)
